US Embassy To Hold Federal Benefits Counseling Sessions Next Week In Medellín & Bogotá, APPOINTMENTS REQUIRED

The U.S. Embassy in Bogota is pleased to announce that representatives from the Regional Federal Benefits Unit will visit Bogota and Medellin to offer services for beneficiaries or individuals with questions about U.S. Social Security and other federal benefits.

As a service to readers, this message has been reprinted in its entirety as issued by the US Embassy in Bogotá.

Tuesday, November 16, 8:30 AM-12:00 PM and 1:00 PM-4:00 PM

Wednesday, November 17, 8:30 AM-12:00 PM and 1:00 PM-4:00 PM

Appointments: Walk-in Services will be available. To schedule an appointment please send an email to [email protected], and in the subject line include: “APPOINTMENT SSA – BOGOTA”. Please bring original documents as well as a photocopy of each document.

Alternatively, you may schedule an appointment by contacting the FBU Santo Domingo at 809-368-7011, Mondays and Thursdays from 8:00 AM to 11:00 AM Colombia time.

WHAT TO BRING FOR SOCIAL SECURITY SERVICES

(please bring legible copies of all documents to be submitted):

Applying for Social Security Benefits:

Bring originals and copies of the following for all applicants: birth certificate, passport, and if applying for auxiliary benefits or survivor’s benefits please bring the marriage and/or death certificates.

Social Security Number Card Application:

Bring a copy of your valid U.S. passport, Certificate of Birth Abroad or original birth certificate and completed form SS-5FS.

Change of Address for Social Security:

Bring your current passport.

Medicare Part B Enrollment/Cancellation:

To enroll in Medicare, you should complete and sign this CMS-40B and to cancel your enrollment please complete and sign this CMS-1763.
